Application Form to the Data Controller

1. General Explanations

Personal data owners defined as "Data Subjects" under the Law on the Protection of Personal Data No. 6698 (“KVKK”) ("Applicant") are granted the right to make certain requests regarding the processing of their personal data pursuant to Article 11 of the KVKK.

This application form has been prepared to determine your relationship with Aysoy Pazarlama Konfeksiyon Tekstil Sanayi ve Ticaret Limited Şirketi (“Aysoy Textile”), to determine your personal data processed by Aysoy Textile, if any, and to respond to your relevant application accurately and within the legal period. In order to ensure the security of your personal data and to prevent unlawful data transfer, additional information may be requested by Aysoy Textile for identification and authorization purposes. In the event that the information provided by the Applicant is incorrect and/or not up-to-date or the requests are unauthorized, the responsibility solely belongs to the Applicant.

Pursuant to Article 7 of the Communiqué on the Principles and Procedures for the Request to Data Controller, if the Applicant's application is to be answered in writing, no fee is charged for up to ten pages. A transaction fee of 1 Turkish Lira may be charged for each page exceeding ten pages. If the response to the application is given on a recording medium such as a CD or flash memory, a fee equal to the cost of the recording medium may be requested.

2. Scope of the Right of Application under Article 11 of the KVKK

The Applicant may apply to Aysoy Textile and make requests on the following issues:

1. To learn whether personal data is processed or not,
2. If personal data has been processed, to request information regarding this,
3. To learn the purpose of processing personal data and whether they are used appropriately for their purpose,
4. To learn the third parties to whom personal data is transferred domestically or abroad,
5. To request correction of personal data in case of incomplete or incorrect processing and to request notification of the transaction made within this scope to third parties to whom personal data has been transferred,
6. Although it has been processed in accordance with the provisions of the KVKK and other relevant laws,

to request the deletion, destruction or anonymization of personal data in the event that the reasons requiring its processing disappear, and to request notification of the transaction made within this scope to third parties to whom personal data has been transferred,

7. To object to the occurrence of a result against the person himself by analyzing the processed data exclusively through automated systems,
8. To request compensation for the damage in case of damage due to unlawful processing of personal data.

3. Method of Application

Pursuant to the first paragraph of Article 13 of the KVKK; applications regarding these rights must be submitted to us in writing and signed, or by other methods determined by the Personal Data Protection Board (“Board”).

In this framework, written applications can be submitted by printing out this form;

• By the Applicant's personal application by hand,
• Via Notary Public,
• By Registered Electronic Mail (KEP) address, secure electronic signature, mobile signature, or
• By e-mail sent from the electronic mail address previously notified to us by the Applicant and registered in our system.

Below, specific information is given on written application channels regarding how written applications will be delivered to us.

Aysoy Textile may request additional information to verify the identity of the Applicant in applications made by e-mail and take necessary precautions.

Your applications submitted to us will be answered in writing or electronically within thirty days from the date of notification or receipt of your request, depending on the nature of the request, in accordance with the second paragraph of Article 13 of the KVKK.
